To all instructors and members of the public training in sessions organised and supervised by the Ophir Karate Association:

1. Every person, regardless of age or sex, shall, upon enrolling as a member of the Ophir Karate Association, inform the same organisation of any pre-existing illness(es) or medical conditions(s), dormant or active, even if they themselves do not consider it/them detrimental to their karate training.

2.  It is incumbent upon all members of the Association to inform their instructor/member of the management committee, of any changes in these circumstances. 

3.  All instructors are required, within a set period of time, to attend first-aid classes organised by the Association or to furnish the Association with a current first-aid certificate from a recognised training agency.

4.  All injuries, no matter how minor, are to reported to the instructor immediately and are to be noted in the accident book. Thereafter, in the first instance, a full written report of the accident must be handed to the management committee.

5.  Emergency first-aid may be administered by the instructor or other qualified person. However, if ANY doubt exists, the condition must be referred to a qualified medical practitioner.

6.  Martins Insurance, Manchester, as the Association's official insurer, must be informed in the first instance, of all accidents regardless of a claim pending or not.
